You'll find all the practical information you need, such as what you can expect during your visit and the forms you will need in order for us to obtain vital information.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Please make sure we have received current dental records from your previous office and bring your current medication list if applicable. We look forward to meeting you!
To expedite your visit, please click 'Get Started' in the white box below to complete and submit your new patient forms. Once received and records are gathered, our New Patient Coordinator will contact you. This may take a few days- If you have an immediate need or are experiencing a dental emergency, please call the office.
*If you are not enrolling in IDC Preventive Plus (our in-house wellness plan for patients without traditional dental insurance), please skip these two forms.
